Is ashwagandha 100% safe?
No responsible clinician will tell you that any bioactive herb is 100% safe for every person, at every dose, in every health context. That includes ashwagandha. The more useful question is whether a standardized, root-only extract such as KSM-66® has a safety record that matches its clinical claims — and who should pause before taking it.
Safety is not a slogan on a bottle. It is a function of extract type, dose, duration, and the person taking it.
Ashwagandha (Withania somnifera) is one of the most studied adaptogens in modern human trials. In randomized, placebo-controlled research, a high-concentration full-spectrum root extract at 600 mg per day has repeatedly shown reductions in perceived stress and serum cortisol, with adverse events that were mild and comparable to placebo. That is not the same thing as a universal safety guarantee. Most published trials last 8 to 12 weeks, enroll screened adults without major liver, thyroid, or psychiatric disease, and use a defined root extract — not an unstandardized powder, leaf blend, or proprietary mix. When those conditions change, the risk profile changes with them.
A dedicated safety trial in 80 healthy adults found no clinically meaningful shifts in hematology, liver enzymes, or thyroid hormones after eight weeks of 300 mg twice daily, and no adverse events were reported in either the extract or placebo arm. (2) A 12-month observational study of KSM-66 at 600 mg/day in 191 adults reported only mild, self-limited events and no serious laboratory signals. (3)
At the same time, case series have linked some ashwagandha products to rare but serious liver injury, and the herb can nudge thyroid hormone output — a benefit in one setting and a problem in another. (4) (5)
This article breaks down what “safe” actually means in the clinical literature, how KSM-66 works on the stress axis, what the human data show, how to read a label, and who should talk to a physician first.
What Is KSM-66 Ashwagandha?
Ashwagandha is a nightshade (Solanaceae) shrub used for centuries in Ayurveda as a rasayana — a rejuvenative tonic traditionally given for vitality, recovery, and resilience under strain. The botanical name is Withania somnifera. The part that matters for modern trials is the root, not the leaf.
The primary bioactive compounds are withanolides, a family of steroidal lactones that appear to modulate the hypothalamic-pituitary-adrenal (HPA) axis, GABA signaling, and inflammatory tone. Standardization to a minimum withanolide content is what separates a clinical extract from a generic powder whose potency can vary from batch to batch.
KSM-66 is a root-only extract produced with a milk-based, green-chemistry process and standardized to ≥5% withanolides. It is the specific form used in more than two dozen human clinical trials on stress, sleep, cognition, and recovery. That manufacturing consistency is not a marketing flourish — it is the reason trial results can be compared at all.
How KSM-66 Ashwagandha Works: The HPA Axis and Why Safety Is Contextual
Most of ashwagandha’s studied benefits — and several of its cautions — run through the same physiology: the body’s stress-response circuit.
When the brain registers chronic threat, the hypothalamus releases corticotropin-releasing hormone. The pituitary gland answers with ACTH. The adrenal cortex then secretes cortisol. In short bursts, that cascade is adaptive. When it stays on, sleep fragments, recovery slows, and inflammatory and thyroid signaling start to drift.
Withanolides appear to dampen this over-firing. In the landmark 60-day trial, adults taking 300 mg twice daily of a high-concentration root extract saw serum cortisol fall by 27.9% versus 7.9% with placebo, alongside large drops in validated stress scores, with only mild adverse events and no serious events. That same HPA and endocrine activity is why ashwagandha is not a candy. If someone is already on sedatives, thyroid hormone, or immunosuppressants — or if the liver is already injured — adding an adaptogen that talks to those systems is a clinical decision, not a lifestyle add-on. (5)
The net result in screened adults at 600 mg/day of a standardized root extract is usually this: lower perceived stress, modestly better sleep, and a side-effect profile that looks a lot like placebo. The net result in the wrong person can be different. That is the honest answer to “is it 100% safe?”
KSM-66 acts on the HPA axis. The same pathway that lowers cortisol is why thyroid, liver, and pregnancy status belong in the safety conversation.
What the Clinical Research Shows
Short-term trials: adverse events look like placebo
Chandrasekhar, Kapoor, and Anishetty randomized 64 adults with chronic stress to 300 mg twice daily of high-concentration full-spectrum ashwagandha root extract or placebo for 60 days. Stress scores fell significantly on every scale by day 60 (P < 0.0001 versus placebo). Adverse effects were mild and similar between groups (6 events versus 5), and no serious adverse events were reported. (1)
Langade and colleagues studied 60 adults with insomnia for 10 weeks at the same 600 mg/day root-extract dose. Sleep onset, sleep efficiency, and PSQI quality improved versus placebo, and the extract was described as well tolerated with no major adverse events. (6)
Dedicated safety labs in healthy adults
Verma, Thakur, and colleagues ran an 8-week randomized, double-blind, placebo-controlled safety study in 80 healthy men and women (300 mg twice daily). There were no reported adverse events and no clinically significant changes in vital signs, blood counts, liver biochemistry, or thyroid hormones compared with placebo. (2) That is strong short-term reassurance for healthy adults — and still not a lifetime warranty.
Twelve-month observational data on KSM-66
A 2025 prospective observational study followed 191 healthy adults, ages 18–65, taking standardized KSM-66 root extract at 600 mg/day for 12 months. Eighteen mild (grade 1) events occurred — nausea, abdominal pain, headache, dizziness, diarrhea, vomiting — all resolved without intervention. There were no serious adverse events and no clinically meaningful deterioration in liver, kidney, or thyroid labs. Serum cortisol declined; lipids and glucose stayed stable. (3)
Observational designs cannot prove causality the way an RCT can, but a year of laboratory follow-up is longer than almost any ashwagandha trial published before it. It supports the idea that a clean, root-only, standardized extract is generally well tolerated when used as studied.
Thyroid function: a benefit that is also a caution
Sharma, Basu, and Singh randomized 50 adults with subclinical hypothyroidism (TSH 4.5–10 µIU/L) to 600 mg/day ashwagandha root extract or placebo for 8 weeks. TSH fell, and free T3 and T4 rose versus placebo, with only mild, transient adverse effects (1 event in the extract group, 3 in placebo). (5)
If you have untreated hyperthyroidism, Graves disease, or you already take levothyroxine, that same thyroid-stimulating signal is a reason to involve your clinician — not a reason to self-experiment.
Rare liver injury: uncommon, not imaginary
A 2023 Indian multicenter case series described 23 patients with ashwagandha-associated liver injury, including eight who had taken single-ingredient products. The typical pattern was cholestatic hepatitis. Injury was often prolonged but self-limited after stopping the product; patients with pre-existing liver disease were at higher risk of acute-on-chronic failure. Product analysis, when available, found phytochemicals rather than obvious adulterants. (4)
These cases are rare relative to how widely ashwagandha is used, and they do not erase the clean laboratory record in controlled KSM-66 trials. They do erase the phrase “100% safe.” Anyone with known liver disease, unexplained jaundice, dark urine, or rising ALT/AST should not start — or should stop — ashwagandha and seek care.
Dosage, Form, and What to Look For on the Label
Across the human clinical trial literature, the effective dose range for KSM-66 ashwagandha is 300–600 mg per day, most often 300 mg twice daily with food. Most trials ran for 8–12 weeks, with participants reporting meaningful stress or sleep change beginning around weeks 2–4. The 12-month observational cohort used 600 mg once daily. (3)
What to look for on the label:
- Ingredient specifically listed as KSM-66® or Withania somnifera root extract (KSM-66)
- Standardized to ≥5% withanolides
- Root extract only (not leaf or whole-plant)
- Minimum 600 mg per daily serving to match the best-studied clinical dose
- Third-party tested (NSF, Informed Sport, or equivalent)
Avoid products that simply list “ashwagandha” without specifying a standardized root extract — these products typically contain inconsistent amounts of withanolides and will not replicate the safety or efficacy outcomes documented in published trials. Leaf-containing extracts can carry a different withanolide profile, including higher withaferin A, and should not be treated as interchangeable with KSM-66.
Safety Profile and Side Effects
In placebo-controlled trials of standardized root extract at clinical doses, KSM-66 has been well tolerated. Serious adverse events have not been a feature of these studies, and laboratory panels for liver, kidney, and thyroid have generally remained stable in healthy adults. (2) (3)
Mild side effects reported in a minority of participants include:
- Mild GI upset if taken on an empty stomach
- Mild sedation (particularly with evening dosing — generally considered beneficial for sleep)
- Nausea, abdominal discomfort, headache, or loose stools
- Rare cases of vivid dreams
- Allergy risk in people sensitive to the nightshade (Solanaceae) family
Take with food or a small snack to minimize GI sensitivity, especially during the first two weeks. Stop and seek medical care if you develop yellowing of the skin or eyes, severe abdominal pain, unusually dark urine, or a marked change in heart rate or mood.
Who Should — and Shouldn't — Take KSM-66
Ideal Candidates
- Otherwise healthy adults with chronic perceived stress who want a clinically studied, root-only extract
- Adults with stress-related sleep disruption who are not on sedative-hypnotics without physician guidance
- Athletes and high-output professionals looking for HPA-axis support at the 600 mg/day dose used in trials
- Adults who prefer a standardized extract with published human safety labs over an unstandardized powder
Who Should Consult a Physician First
- Pregnancy and breastfeeding: Human safety data are insufficient. Traditional texts and modern reviewers advise against use in pregnancy.
- Known liver disease or unexplained enzyme elevations: Rare herb-induced liver injury has been documented; pre-existing liver disease raises the stakes. (4)
- Hyperthyroidism or unstable thyroid disease: Root extract can increase T3/T4 and lower TSH. (5)
- Autoimmune conditions and immunosuppressant therapy: Ashwagandha can influence immune tone; coordinate with the prescribing clinician.
- Hormone-sensitive prostate cancer or androgen therapy: Some data show modest testosterone increases with long-term use. (3)
- Sedatives, thyroid hormone, or diabetes medications: Additive effects on sleepiness, thyroid labs, or glucose are possible.
- Nightshade allergy: Ashwagandha is in the Solanaceae family.
- Children: Adult trial doses should not be extrapolated without pediatric guidance.
