Do ashwagandha have side effects?
People do not usually ask about ashwagandha because they want a scare story. They ask because a friend swore it calmed them down, a bottle promised stress relief, and then a search result mentioned liver injury, thyroid swings, or stomach pain. The honest answer is yes — ashwagandha can have side effects — and the more useful answer is that most of those effects, in human trials of standardized root extracts such as KSM-66, are mild, uncommon, and dose-dependent, while a smaller set of people should not take it at all.

Side-effect conversations go wrong when every ashwagandha product is treated as identical. A tea, a whole-plant powder, a leaf-containing extract, and a root-only extract standardized to withanolides are not the same intervention. Clinical safety data cluster around branded root extracts. Case reports of more serious harm often involve products whose composition, dose, and contaminants were never verified the way a trial material is verified.
In the landmark 2012 randomized trial of high-concentration full-spectrum ashwagandha root extract (KSM-66), 300 mg twice daily for 60 days reduced perceived stress and lowered serum cortisol by 27.9% versus 7.9% on placebo, with adverse events described as mild and comparable between groups. (1)

These molecules appear to influence the hypothalamic-pituitary-adrenal (HPA) axis, GABA-related calming pathways, and selected inflammatory and thyroid markers. Withanolide content is the usual standardization target because raw root powder can vary widely from batch to batch.
KSM-66 is a full-spectrum root extract made without alcohol, using a process that retains the native withanolide profile rather than isolating a single molecule. It is standardized to ≥5% withanolides by HPLC, is root-only, and has been used at 300–600 mg per day in published human trials on stress, sleep, strength, and sexual function. What the clinical research shows about side effects
Stress trials: adverse events look like placebo
Chandrasekhar, Kapoor, and Anishetty randomized 64 adults with chronic stress to 300 mg KSM-66 twice daily or placebo for 60 days. Stress scores fell across standard scales, and serum cortisol dropped 27.9% from baseline. Safety monitoring found only mild events and no serious adverse events; laboratory values did not shift in a clinically meaningful way. (1)
Lopresti and colleagues later tested 240 mg/day of a standardized ashwagandha extract for 60 days in stressed adults. Anxiety and morning cortisol improved versus placebo. The extract was described as well tolerated, again without a safety signal that would distinguish it from placebo in a meaningful way. (2)
Dedicated safety study in healthy adults
A 2021 randomized, placebo-controlled safety study in healthy volunteers specifically asked whether ashwagandha root extract disturbed thyroid, liver, hematologic, or vital-sign parameters. Over 8 weeks, researchers reported no significant abnormalities in liver enzymes, thyroid panel, blood counts, or vitals compared with placebo. That paper is one of the cleaner answers to "is this herb quietly toxic at clinical doses?" — in healthy adults, at a studied root-extract dose, the laboratory answer was no. (4)
Sleep studies: sedation is often the point
Langade et al. evaluated 300 mg of ashwagandha root extract twice daily for 10 weeks in adults with insomnia. Sleep onset, sleep efficiency, and sleep quality improved versus placebo. Somnolence in this setting is frequently listed as a side effect and, for people taking the herb at night, as a desired effect. The distinction is timing and context, not a hidden toxicity. (5)
Thyroid data: a real effect, not a rumor
Sharma, Basu, and Singh studied 600 mg/day of ashwagandha root extract for 8 weeks in patients with subclinical hypothyroidism. Serum TSH fell and T3 and T4 rose significantly versus placebo. (3) That is evidence of a genuine endocrine effect. It is also the reason people with known hyperthyroidism, Graves disease, or unstable thyroid replacement should not self-experiment.
Rare liver injury: case reports, not trial tables
A 2023 case series from India and a growing LiverTox file document idiosyncratic ashwagandha-associated liver injury. Presentations have included jaundice, itch, and mixed or cholestatic enzyme patterns, typically weeks to months after starting a product. Most published cases improved after stopping the herb; a minority were more severe. (6) These reports matter. They do not mean typical KSM-66 trial participants developed hepatitis. They mean ashwagandha is not risk-free in every host, every product, and every unregulated dose — and anyone who develops unexplained fatigue, dark urine, pale stools, or yellowing of the skin should stop the supplement and get labs.

Dosage, form, and what to look for on the label
Across the human clinical trial literature, the effective dose range for KSM-66 ashwagandha is 300–600 mg per day, often given as 300 mg twice daily or 600 mg once daily. Most trials ran for 8–12 weeks, with participants reporting meaningful improvement beginning around 2–4 weeks. Going far above the studied range does not have a matching safety file, and it is one of the easier ways to manufacture a side-effect story.

Mild side effects reported in a minority of participants include:
- Mild GI upset, nausea, or loose stool if taken on an empty stomach
- Mild sedation or next-morning grogginess (particularly with evening dosing — often considered useful for sleep)
- Rare cases of vivid dreams or headache
- Transient increase in appetite or a heavier, "slowed down" feeling in the first days
Take with food or a small snack to minimize GI sensitivity, especially during the first two weeks. If drowsiness is unwelcome, move the capsule to evening. If drowsiness is the goal, do not stack ashwagandha with alcohol, sedative medications, or other strong calming herbs without medical advice.

- Hyperthyroidism or unstable thyroid disease: Root extract can raise T3/T4. Combining it with thyroid hormone or taking it in Graves disease is a medical decision, not a wellness experiment.
- Known liver disease or unexplained enzyme elevations: Idiosyncratic injury is rare, but a damaged liver is the wrong place to test an herbal unknown.
- Hormone-sensitive prostate cancer or androgen-related conditions: Some data suggest ashwagandha can influence testosterone and related markers. Get specialist clearance.
- Autoimmune disease on immunosuppressants: Theoretical immune-stimulating effects mean this should be cleared with the prescribing clinician.
- Upcoming surgery or sedative / thyroid/diabetes medication: Additive sleepiness and possible effects on glucose and thyroid labs require a plan to pause or monitor.
